The Department of Otolaryngology - Head and Neck Surgery utilizes REDCap (Research Electronic Data Capture) to effectively manage research studies. Please follow the links and detailed instructions below:

• Study proposal - Fill out this form for new study proposals, i.e. initiating a new study. Once you submit the form, a new page will open that says "Thank you for taking the survey". On this page is a return code. Copy and paste this code into an email to obtain signatures from your faculty PI and research director. 
• Study status and participation - Fill out this form for study opportunties after obtaining approval to initiate a new study. Upon form submission, copy and past the return code provided into an email to the research director. This will help us keep tabs on ongoing studies that have been approved.
• Study presentation and manuscript - Fill out this form for new study presentations and manuscript submissions. Upon submission, copy and paste the return code provided into a new email to your faculty PI and research director to obtain signatures. 
• Study completion - Fill out this form when your study is completed and ready to be closed out. Upon submission, copy and paste the return code provided into an email to your faculty PI and research director to obtain signatures.
